Most older Australians want to stay in their own homes for as long as possible, close to the people, places, and routines that are meaningful to them.  

This module provides an overview of the Support at Home program’s purpose, structure, and key features. You’ll explore how people access services, how care is delivered and managed, and the important role you play in supporting older Australians to maintain independence, safety, and dignity at home. 

By completing this module, it is anticipated that you will be able to:  

  • Explain the key features and aims of Support at Home
  • Describe service access, care delivery, and care management  
  • Identify roles and responsibilities of those involved in the older person’s care
  • Recognise the features of a safe, rights-based approach to support for older people living at home
